A Holistic Approach

Personalised care planning is essentially about addressing an individual’s full range of needs, taking into account their health, personal, family, social, economic, educational, mental health, ethnic and cultural background and circumstances. It recognises that there are other issues, in addition to medical needs, that affect a person’s total health and well-being. It is therefore a holistic process, treating the person “as a whole” with a strong focus on helping people, together with their carers, to achieve the outcomes they want for themselves. In doing this, it should open up more choices that are relevant for people with long term conditions.

A BREAK WHEN YOU NEED IT

Respite, or short-term care, offers an opportunity to sample life at Hazelwell Lodge slowly introducing the peace of mind and security we provide without any commitment to a long-term arrangement. Our flexible respite care arrangements often prove to be invaluable for older people and their families. For individuals managing at home alone, a short stay provides a break from the stresses of coping alone. For those who require more support respite care can provide full-time carers with a well-deserved holiday, safe in the knowledge that their loved one is happy and in safe hands.

SPEND SOME TIME WITH US

We understand that for many individuals there may come a time when, for those able to live in their own homes or with family, it can be difficult to find the right level of support and activity, particularly for those facing the challenges associated with the symptoms of dementia or reduced mobility.

We offer a day care facility to support those who may require it and their families to assist them in remaining independent and active for as long as possible. Our day care service can help to bridge the gap between an independent life at home and a move into permanent residential care. We offer a day care option to enable us to assist you to choose an arrangement that meets all your care needs.

Day care residents are welcomed into Hazelwell as ‘part of the family’ and we find that, in addition to the benefits of companionship, genuine friendships are often formed between our permanent residents and those enjoying day care. We are also able to provide day care residents with a wide range of activities, from arts and crafts and gentle exercise to trips out to places of local interest. Delicious home-made and nutritionally balanced meals are, of course, provided along with morning coffee, afternoon tea and the opportunity to sample our cooks’ fabulous home baking! Personal care services such as chiropody and hairdressing are also available.

If you have any questions or would like to arrange a visit, please call us for free using the click to call button. Alternatively you can get in touch using the email form on our Contact Us page.
